Credit/Debit Card Authorization

This form is required at the time of booking to guarantee payment for services rendered by The Perfect Maid and Nanny. All information will be kept strictly confidential and securely stored in accordance with data protection standards.

AUTHORIZATION & TERMS

1. You authorize The Perfect Maid and Nanny (TPMN) to securely retain your card information on file for the sole purpose of processing payments for services rendered by TPMN.

2. Charges will only be applied to your card after an official invoice has been generated and issued to you.

3. This authorization will remain in effect until written notice of cancellation is provided by you to TPMN, and any outstanding balances have been settled.

4. You agree to notify TPMN immediately of any changes to your card details or if the card is cancelled or replaced.

5. TPMN agrees not to disclose your card information to any third party and will only use it in accordance with the terms outlined in this agreement.

6. If payment is declined after services have been rendered, you agree to: - Provide an alternate form of payment within 24 hours of notification, - Accept full responsibility for the outstanding balance, - Understand that failure to settle the amount due within this timeframe may result in suspension of services and/or referral to a collections agency
