This clause outlines the informed consent and waiver requirements for The Perfect Maid and Nanny’s babysitting services:

 

1. Informed Consent:
By engaging The Perfect Maid and Nanny’s babysitting services, the undersigned parent or legal guardian hereby provides informed consent for the care and supervision of their child(ren) by our caregivers.

 

2. Acknowledgment of Policies:
The undersigned acknowledges receipt and understanding of The Perfect Maid and Nanny’s Service Agreement, including those related to child safety, emergency response, and service guidelines.

3. Assumption of Risks:
The undersigned understands and assumes any inherent risks associated with childcare services, recognizing that despite all reasonable precautions, unforeseen circumstances may arise.

4. Release and Waiver of Liability:
The undersigned releases The Perfect Maid and Nanny, its owners, employees, and caregivers from any liability for injuries, accidents, or incidents that may occur during the provision of babysitting services, to the fullest extent permitted by law.

5. Emergency Medical Treatment Authorization:
In the event of a medical emergency, the undersigned authorizes The Perfect Maid and Nanny’s and its caregivers to seek emergency medical treatment for the child(ren) named herein. The undersigned also agrees to be responsible for any associated medical expenses.

6. Revocation of Consent:
The undersigned reserves the right to revoke this consent at any time by providing written notice to The Perfect Maid and Nanny. However, such revocation may impact the availability of babysitting services.

By signing below, the undersigned parent or legal guardian acknowledges that they have read and understood this Informed Consent and Waiver Clause and agree to its terms and conditions.
